Open enrollment for 2018-2019 will be from May 1, 2018 through May 13, 2018. No existing benefit elections will rollover into the coming 2018-19 insurance plan year.

No existing benefit elections will rollover into the coming 2018-2019 insurance plan year.

 

“Do I have to re-enroll in all of my benefits if I’m not making any changes?”

 

Yes. You must re-enroll in all benefits you wish to participate in, even if you are keeping all of your plan choices the same as they are now. This includes: Medical, Dental, Flexible Spending and your Life Insurance Beneficiary.

 

Even if you just filled out enrollment forms as a new enrollee in the last few weeks, you will still need to enroll in your benefits again now during Open Enrollment.

 

If you do not re-enroll in all benefit plans you want by the close of the 2018 Open Enrollment period (11:59PM CST on 5/13/18) your Medical, Dental and Flex benefits will terminate as of the close of the ’17-’18 plan year.

 

Even if you are waiving all benefits, you must still complete the Insurance Understanding Form to indicate that you are waiving enrollment.

These are your enrollment steps to ensure you do not miss anything in the enrollment process:

Once you are to the Welcome page, please select – Create an account

You will be required to create a username and password.   Once you have created a username and password, you will have access to the enrollment platform.  Once you are ready to enroll, click the “Enroll Now!”  button to begin the enrollment process. 

 

Reminder – The Benefits Office will have a number of sessions available  for employees  to drop in and ask questions or receive help enrolling online.  These sessions are listed below.

Voluntary Vision

 

 

Effective 7/1/17, Hopkins will offer Voluntary Vision Insurance to employees who meet the same eligibility requirements to participate in Hopkins Medical Insurance. This plan can help subsidize and/or cover the cost of glasses, contact lenses and contact lens fittings.

 

This plan is voluntary and there is no employer contribution toward the monthly premium cost. Employees who elect to enroll in Vision Insurance will pay the full premium cost of the plan selected via payroll deduction (see Learn More).

 

Note: The Hopkins Medical Plan through PreferredOne will still cover your annual eye exam. The Voluntary Vision plan through Superior Vision does not cover annual eye exams.

Flexible Spending

 

 

 

The IRS has increased the annual limit an employee may contribute into his/her Medical Flex Spending account from $2,550 annually to $2,600 annually ($50 increase). The Dependent Flex limit remains the same at $5,000 annually.

 

Employees who enrolled in Medical Flex Spending for the current plan year (17-18) should be sure to submit  medical claims to Genesis before 6/30/18. Up to $500 of Medical Flex dollars will rollover from the 17-18 plan year into the 18-19 plan year. Any Medical Flex dollars above $500 in an employee’s Medical Flex account after 6/30/18 are lost and not paid back out to the employee.Do not wait until 6/30/18 to submit your Medical Flex claims. Submit them as soon as possible to avoid losing your elected Flex dollars.

Hopkins Medical Privacy Notice

 

The Hopkins medical insurance plans through PreferredOne have no significant plan changes from the current year into the ‘18-‘19 plan year.

 

The monthly premiums for the plans are staying the same, as listed below (does not include any employer contribution):

 

  • Low Deductible Single: $699 per month
  • Low Deductible Family: $1,352 per month
  • High Deductible Single: $582 per month
  • High Deductible Family: $1,122 per month

Dental

 

 

 

The Hopkins dental insurance plan through HealthPartners has no significant plan changes from the current year into the ‘18-‘19 plan year.

 

The monthly premiums for Hopkins dental insurance plans are staying the same, as listed below (does not include any employer contribution):

 

  • Single Dental: $44 per month
  • Composite Dental: $86 per month
  • Family Dental: $104 per month

 

To determine how much Hopkins will pay per month toward the plan cost above, please see the Insurance section of your Bargaining Contract.

 

Note: All Teachers and District Support Staff who enroll in dental insurance are required to be on the Composite plan regardless of if the election is single or family.

Basic Life Insurance

 

 

 

The Hopkins basic life insurance plan through Guardian has no significant changes from the current year into the ‘18-‘19 plan year.

 

Your enrollment in Basic Life Insurance is automatic so long as you meet the eligibility threshold based on your bargaining group. Your basic life insurance coverage amount is also based on your bargaining group.

Voluntary Life Insurance

 

 

 

 

Employees who meet the eligibility requirements for the basic life plan may apply for additional life insurance for self, spouse and children. Applying for supplemental life insurance coverage may require a medical screening at the discretion of Guardian’s life insurance underwriting team. If the supplemental life insurance application is approved by Guardian, you will be contacted by HR during which time the per paycheck cost will be given to you and you will decide yes or no whether you wish to proceed with that supplemental enrollment for life insurance.

403b Retirement Account

 

 

 

 

 

Employees have the option to contribute to a 403b plan. To do so, you must first setup a 403b account with one of our two carrier options, Fidelity or MetLife.

 

To enroll through Fidelity, click the Enroll button below and follow the instructions on the guide. To enroll through MetLife, click the Enroll button below and contact Anita Hawks for assistance setting up the account.

 

Once you have an active Fidelity or MetLife account per the step above, complete the 403b Salary Reduction Agreement and send it to HR to be processed by Payroll.
