Immunization Information from NJH's School Nurse

Important information from our school nurse regarding required immunizations and our medication administration policy.

Immunizations:  According to MN School Immunization laws, your child has to have the required immunizations to start school. In-coming 7th grade students are required to submit documentation of a recent Tdap (tetanus, diptheria, pertussis) two Varicella, and one Meningitis vaccines along with their shot record. Elementary schools began to collect the documents in the Spring and those will be transferred to North Junior High. If your student has not already received the immunizations, we recommend you contact your healthcare provider immediately. Parents may file a medical exemption signed by a healthcare provider or a non-medical exemption signed by a parent/guardian and have it legally notarized. If needed, our school nurse, Ann Salzer can complete a referral for your student to receive free immunizations. All health forms and immunization records need to be received by August 1, 2017. Please note: Your child will NOT be given their class schedule in August until North Junior High has an up-to-date written immunization record on file.

Medication Administration at School: Parent/Guardians asking school staff to give prescription medications to their child must provide written permission from themselves and their healthcare provider. Nonprescription medication will be given on a short term basis with written permission from a parent/guardian. Per district policy, all medication must be kept in the nurse's office. Attention All Athletes!

Thinking of playing in an after-school sport this year? A sports physical is requires to be on file every three years. Free exams can be obtained at the Central Clinic in St. Louis Park. Get the 2017-18 Sports Physical Form here
