To enroll your child in elementary school, we need the following:
❐ Proof of residence—Valid MN Driver's License or ID card with correct address; Utility bill delivered within last 30 days; Original Copy of Current Lease/Rental Agreement or Purchase agreement for home
❐ Student Enrollment Form
❐ Release of Records (Consent for Current School to send records)
❐ Immunization Record—The immunization records are required for school entrance
❐ Student Health Information Form
❐ Free/Reduced Lunch Form–Used for Scholarships and Lunches, Available in August
All the enrollment materials are available as downloads in the resources box or from any of our elementary school offices during the school year. During the summer—June 15 through August 15—enrollment packets are available at our District Office reception desk (1001 Highway 7, Hopkins).

Once enrollment and immunization forms are received, the elementary school will be contacting parents to register the student into classes.
