All the information and forms to register your child for kindergarten.

All the registration materials for kindergarten are available as downloads on this page, from any of our elementary school offices, or at your child's Kindergarten Roundup session. During the summer—June 15 through August 15—enrollment packets are available at our District Office reception desk (1001 Highway 7, Hopkins).

To enroll your child in kindergarten, we need the following:

❐ Student Enrollment Form—This form registers your child for school.

❐ Consent for Health Services

❐ Emergency Contact and Health Information

❐ Student Health Inventory Form

❐ Physical Examination —The physical examination is recommended. 

❐ Immunization Record—The immunization records are required for school entrance.

❐ Documentation of Early Childhood Screening, if completed in a district other than Hopkins.  (If you had the screening in our district, we automatically receive the records.)  To schedule a screening, call 952.988.5017.

❐ Birth Certificate—Legal proof of your child's birth date is required at the time of registration. An original or notarized copy of your child's birth certificate will be photocopied and kept in their health file. Please bring the certificate to Kindergarten Roundup, or to the school office before the first day of school.  Information on how to obtain your child's birth certificate may be found at the link below.
